The Abrazo Western Neuro Central, located in Phoenix, Arizona, in partnership with Western Neuro in Tucson Arizona, is a statewide comprehensive Neurosurgery program offering services that meet the increasing needs of our communities. Our promise is to partner with each patient by utilizing our skilled team of attending physicians and providers to ensure seamless care coordination and to safeguard the best interest for each patient. We have board certified Neurosurgeons with the unique ability and training to provide patients with a â��one stop shopâ�� for a variety of neurovascular, neuro-oncology and spinal conditions. We highly value each patientâ��s time and work effortless to ensure that no patient is waiting for an extended period of time to be seen.
Provide pre-operative education to patients for diagnostic procedures and/or recommended surgery, including brochures and discharge instructions to help patientâ��s prepare for the post-operative period.
Obtain pre-operative clearance and labs from primary care doctor and/or patientâ��s specialty doctors.
Recognizes data from simple to complex situations to determine priorities of care; includes appropriate physical, psychosocial, educational, and safety needs.
Assist in coordination of resources required for aiding in recovery post-operatively, including social work, primary care providers, and specialty doctors.
Review lab results and risk stratification completed by primary care and specialty doctors, and discuss critical lab results and moderate-to-high risk stratification with provider.
Triage post-operative patients and coordinate care appropriately based on clinical judgment.
Manage and respond to patient calls in a timely manner.
Monitor abnormal vital signs reported by the MA and relay findings to patientâ��s primary care doctor.
Assist in admitting patient directly from clinic into the hospital.
Facilitates timely scheduling of appointments, diagnostic testing, and procedures in coordination with the patient care coordinator in order to expedite the plan of care and to promote continuity of care.
Facilitates communications among members of the care team to prevent fragmented or delayed care that could adversely affect patient outcomes.
For newly diagnosed brain tumors, coordinate follow-up with oncology in a timely manner.
Call patient within 7 days of discharge to ensure that they are successful in their recovery period.
Perform 10-14 day incision checks post-operatively, and report any signs or symptoms of infection to provider
