M-Health Fairview is looking for a Cancer Survivorship Coordinator to join our team. The goal of the Survivorship Program at M-Health Fairview is to help coordinate care for cancer survivors, and to prevent and manage late effects from cancer treatment. This position is a full-time day position, mobile working status, no weekends, no on-call and benefit eligible.

The Survivorship Coordinator is responsible for preparation and delivery of survivorship care plans for adult oncology patients and for the identification and management of long-term survivorship needs of patients in the post treatment phase of cancer care through collaboration with various members of the interdisciplinary team. The primary focus is to promote health, wellness and improved quality of life for cancer survivors during and after cancer treatment and to assist front-line personnel with coordinating supportive care interventions that support patient safety and high-quality outcomes.

• Assists in the development, maintenance, and delivery of individual treatment summaries and care plans for patients in accordance with applicable standards, policies, procedures, and guidelines within the Cancer Care Service Line.
• Supports and encourages oncology patients to participate in survivorship services.
• Participates in committee work and data collection and analysis for survivorship program. Provides reporting to constituencies as indicated. Communicates with the Cancer Care Service Line disease teams and other departments outside the service line who provide service for oncology patients to encourage knowledge and utilization of survivorship program.
• Assists in the implementation and follow-through of cancer program accreditation requirements and cancer care service line strategic objectives, policies, and processes.
• Collaborates with other members of the clinical team to improve patient care and benchmarks through quality improvement projects
• Brings creative solutions to assist in the development and growth of the Cancer Care Service Line by assisting in the planning, organization and implementation of services offered to clients.
• Maintain knowledge of current practices in oncology, nursing, and healthcare.
• Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served

• Performs other duties as assigned

Required

• BA/BS in Nursing
• MN RN License
• Oncology Nursing Certification upon hire or must achieve within 2 years of hire.
• Three years oncology nursing experience
• Excellent communication skills, both written and verbal
• Possess good analytical and deduction skills
• Organized and able to multi-task while working in a fast-paced environment, with a positive attitude
• Proficient in Microsoft Office and the EHR.
