Overview M-Health Fairview is looking for a Cancer Survivorship Coordinator to join our team. The goal of the Survivorship Program at M-Health Fairview is to help coordinate care for cancer survivors, and to prevent and manage late effects from cancer treatment. This position is a full-time day position, mobile working status, no weekends, no on-call and benefit eligible. The Survivorship Coordinator is responsible for preparation and delivery of survivorship care plans for adult oncology patients and for the identification and management of long-term survivorship needs of patients in the post treatment phase of cancer care through collaboration with various members of the interdisciplinary team. The primary focus is to promote health, wellness and improved quality of life for cancer survivors during and after cancer treatment and to assist front-line personnel with coordinating supportive care interventions that support patient safety and high-quality outcomes.
