Overview Fairview Home Infusion is a nonprofit provider with over 25 years of experience, offering personalized home infusion care for adults and children of all ages. Our mission: Helping people of all ages manage their illness while continuing to live their lives. We care for patients across the five state areas of Minnesota, Wisconsin, Iowa, North Dakota and South Dakota. We are looking for a full time Nurse (RN) Liaison. This is a benefit eligible, weekday position. This position involves travel (mileage is reimbursed). This position will float to all of the M Health Fairview hospital sites. The Nurse (RN) Liaison develops referral source relationships to ensure smooth patient-centered transitions to Fairview Home Infusion. The Liaison provides home infusion education and expertise to discharge planners, care coordinators, patients, families and other members of the care team. This may include attending care conferences, rounds and assisting in problem solving complex cases. As the Nurse Liaison you will provide education and support to patients and caregivers. Enable patients to safely and effectively self-administer medications and solutions. Coordinate the discharge of patients with the care coordinators, physician and other disciplines to achieve an orderly transition for the patient/family. Participate in the sales plan for assigned areas. Coordinate effort with appropriate leadership and sales teams. Participate in conversations with physicians, clinic managers, social service departments, discharge planners, program specialists, care coordinators, third party payers and their case managers, and specific employers. Work directly with the operations team and managers and the marketing staff to meet promotional and advertising needs. Promote Fairview Home Infusion clinical services and programs to key referral sources. Serve as a resource to healthcare facility staff regarding home infusion services. Maintain appropriate records and providing education. Build relationships to thoroughly understand the needs of customers, patients, referral sources and Fosters the relationship between patients and the facilities providing their care. Communicate with families, and interacting with a wide range of staff members, from admissions coordinators to case managers to physicians. Influence and motivate others to action, pulling them together, coordinating projects, resources and people to achieve implementation. Act as an active sales team member contributing to identifying and meeting needs of each business area. Required:

Proficient in the English language, good attention to detail, ability to problem-solve effectively, ability to follow policies and procedures, basic math skills, and basic computer skills.
