Overview Fairview Home Infusion is a nonprofit provider with over 25 years of experience, offering personalized home infusion care for adults and children of all ages. Our mission: Helping people of all ages manage their illness while continuing to live their lives. We care for patients across the five state areas of Minnesota, Wisconsin, Iowa, North Dakota and South Dakota. Fairview Home Infusion staff provides one-on-one home infusion nursing care visits and patient education in accordance with the physicians plan of treatment. Responsibilities include teaching patients about their home infusion therapy, administering IV therapies and providing ongoing monitoring of their treatment. Fairview Home Infusion seeks RNs who are customer-service oriented and seeking growth. The Casual Visit Nurse Clinician in Home Infusion will coordinate the delivery of all care and services appropriate to the needs of the patient of all ages and their families using an interdisciplinary team approach. Complete a thorough physical and psychosocial assessment of the patient and identify effective interventions with the patient and family under the direction of the physician to address the specific care goals and formulate a Plan of Care. Provide necessary education and support to patients and caregivers while performing ongoing case management. Monitor progress toward established goals to achieve optimal outcomes. Maintain complete, timely and accurate clinical documentation for each patient. Provide clinical supervision to other professional/paraprofessional staff as appropriate. Utilizes clinical expertise and advanced nursing skills in providing care/services. Minimum Requirements: Graduate of an approved school of professional nursing. Current RN licensure in the State of Minnesota Current BLS certification Minimum of two years of recent nursing experience in an acute care setting, preferably in critical care, oncology or med/surg. Preferred Requirements: BSN preferred, or equivalent in a health related field. Certification in infusion therapy, oncology or critical care. One to two years of previous home care/home infusion therapy experience. Infusion therapy experience in an acute care setting or ambulatory care setting. Experience with pediatrics, geriatrics, and oncology patients. Additional Requirements: Must be licensed to operate a motor vehicle. Must carry own automobile insurance. Considerable physical energy required related to driving and providing care in the home to patients. Must be available to rotate weekends. Excellent organizational and documentation skills, orientation to detail and ability to handle multiple tasks in a fast paced environment. Excellent communication and interpersonal skills. Demonstrated commitment to excellent customer services and service excellence. Must be able to work independently and also be a supportive team member.
