This is unique opportunity to be at the forefront of a pioneering new model of acute oncology care in the rural communities of Utah bringing high level intensive support to the Huntsman Cancer patient's who live in remote rural locations. Partnering with local Home Health agencies to help the delivery of this care, the NP role will be to lead and oversee the care and will require travel to the rural Utah counties of Grand, Emery and Carbon. Dealing with cancer can be difficult enough but living at a distance from the cancer center adds to the challenge. Two years ago, the Huntsman Cancer Institute began an innovative home-based acute care program, Huntsman at Home, for cancer patients in the Salt Lake City are. This very successful and well received program, reduces the need for unplanned hospitalizations and emergency department visits that sometimes happen due to side effects from the treatment and disease. this will be a nurse practitioner lead program, and will be a combination of actual visits, supplemented by virtual visits using telehealth and tele monitoring.
