Fairview Home Care & Hospice is one of the Twin Cities largest home health care agencies. We have been in business more than 30 years. Our staff is known for their excellence in patient care and can meet the needs of patients, from basic care to the most complex. We are looking for aHospice RN to travel throughout the Princeton, MN and surrounding areas to work in patients homes and facilities.
This is casual position (opportunity to pick up shifts ahead of time or last minute) and requiring one weekend a month.
Ongoing development and implementation of the plan of care
Coordination of services
Maintains complete, accurate, legible and timely clinical records/ documentation on all patients according to agency policy
Provides safe, effective skilled and advanced intervention, within scope of practice according to patient need.
Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served
Fulfills all organizational requirements
Performs other duties as assigned
Stays current (expert) in home care regulation and all applicable licensing
Actively participates in educational programs, meetings and agency committees as appropriate
Accepts responsibility for the understanding and implementation of the discipline applicable content of all departmental communications and procedures
Identifies processes to improve and potential solutions
One year recent RN experience
Ability to function in job role independently
Computer skills: able to document effectively in EMR
Licensed as a RN and eligible to practice in Minnesota
Current CPR certification (BLS for Healthcare Providers)
Valid Minnesota drivers license (or license that matches your current permanent residence).Current and adequate automobile insurance policy
Bachelors degree in nursing
Two or more years RN experience.
RN experience in oncology, med/surg, critical care or high tech home care/ hospice
