JOB TITLE:Heart Failure Nurse Coordinator LOCATION: Geisinger Medical Center Hospital DEPARTMENT:Cardiology WORK SCHEDULE: Days WORK TYPE: Full time
JOB SUMMARY: Functions in an expanded nursing role within the Urgent Heart Failure clinic at Geisinger Medical Center in Danville. Responsible for assisting the Physician Director and Advanced Practitioners in providing direct patient care to patients in the Heart Failure Clinic, as well as those hospitalized for their illness. Responsible for the coordination of patient care from the inpatient setting to an outpatient setting in order to improve outcomes. Serves as the liaison between the patient and caregivers during hospitalization, immediate post-discharge period, and ongoing follow up in the outpatient setting. Coordinates appropriate follow-up post hospital discharge.
Works collaboratively under the clinical direction of the physician and advanced practitioner members of the multi-disciplinary team and the administrative direction of the Regional Operations Director of Cardiovascular Services. Collaborate with Physician Director and Chief Nurse Practitioner in the growth and development of the Urgent Heart Failure clinic.
Performs duties to provide the highest level of patient care and satisfaction.

EDUCATION AND/OR EXPERIENCE: Registered nurse currently licensed in PA required, BSN required.
Minimum two years of clinical experience in caring for cardiovascular patients required.
Experience in heart failure care preferred.
Maintenance of BLS/ACLS certifications required.
CHFN certification preferred within two years.
WORKING CONDITIONS/PHYSICAL DEMANDS: Work is typically performed in a clinical environment. Some travel required.
