Responsible for the intake of referrals and to act as lead in patient care and coordination across the continuum for Infectious Diseases and Rheumatology patients. Coordinates outpatient clinic and infusion visits, and ensures testing and results are available at the time of the clinic visit. Performs as a point person for patient contact, and as a patient advocate in the medical care system, serving as confidante, advisor, and liaison between patients and physicians. Assists with inpatient needs as appropriate. Meets the qualifications to provide care for patients in the specific age range on their assigned clinical area.
Performs duties to support the goals and objectives of the Geisinger Health System in its efforts to provide quality patient care and the highest level of patient experience.
Reports to clinic supervisor. Works under the administrative direction of the operations manager and the clinical direction of the Infectious Diseases and Rheumatology physician directors.

Computer literacy required. Preferred applications: MS office, Outlook, EPIC
EDUCATION AND/OR EXPERIENCE:
Bachelor's degree in Nursing or equivalent, with current Pennsylvania licensure, required. Master's degree preferred.
Clinical experience required.
Experience with Rheumatology and Infectious Diseases preferred.
BLS certification required
